ACT Clutch/FW questions

OK folks. I have a Stg2+ '05 LGT. I installed the ACT LWFW/HD street disc set up about 21 months ago.

The car has not been raced, the clutch was ridden once for about all of 5 seconds about 18 months ago.

 

I recently had a bearing go bad in the tranny that Subaru replaced. Ever since then, I notice after about 30mins of driving with some stop-n-go, that the clutch will shudder when starting off in 1st. Once it starts it gets progressively worse.

 

1. Is this uncommon premature wear of the assembly?

2. Could a small amount of lubricant on the surface of the disc/FW/PP cause this?

3. Is this a symptom of a warping FW/Disc?

4. Can the LWFW be machined if no longer true (yes I know there are limits if really bad)?
